Today it covers around 876,000 members and pulls in roughly £200m in annual revenue. But Tom isn’t just playing for history points. His goal is clear and ambitious, hit one million members. One million signals that this secret giant is not just surviving – it’s finally being seen.

In a healthcare system under mounting strain, Tom’s task is to take a heritage brand with deep roots and reposition it as a modern, challenger alternative that sits somewhere between state provision and traditional private medical insurance.

Tom has been in post for about 15 months and describes himself as an outsider in more ways than one. “This is my first time in healthcare,” he said, and his first time leading a mutual.

He started out in engineering. Sponsored by Rolls-Royce, he quickly realised he was less enthused by machines than by people. “Within three months of joining them, I realised that I couldn’t stand the engineering. It was too slow, very technical. And the bit I really loved was working with people.”

Consulting led him into financial services, including a baptism of fire at Halifax Bank of Scotland (HBoS) shortly after its merger and in the shadow of the financial crisis.

“My first job was at HBoS,” he recalled. “I joined them three months after the merger, was there through the financial crisis – very interesting, leading a business that was effectively failing and having to motivate and engage staff.”

From there he ran the UK general insurance arm of Direct Line Group, worked in start-ups and private equity-backed businesses, and built a reputation around transformation, customer focus and commercial change.

It is precisely that external perspective the board appears to have wanted. Benenden, for all its scale, has historically been cautious, steady and – as Tom acknowledges – largely invisible outside its traditional constituencies.

Central to Benenden’s story is its structure. Unlike big-name insurers such as Bupa, AXA, and Aviva, it is not an insurer at all. “We are a discretionary provider, so as a mutual our members pay a monthly subscription, and that doesn’t vary depending on how much you’ve claimed, your age, or your risk. You contribute into the pool, and the directors and executive set the benefits we offer.”

That model keeps pricing simple and accessible. “Fundamentally, we provide diagnostic services, surgical treatments, GP access, physios, but we do it differently,” he added. “As a mutual – owned by our members and not for profit – our goal is to make healthcare affordable and available to the largest population possible.”

In Tom’s words: “Benenden sits somewhere between the NHS and those private insurers.” Its products are pitched as “affordable, high quality” and designed for the whole workforce, not just senior executives. The positioning is deliberate. “We focus on complementing the NHS,” he explained. “There’s pride in the NHS, but it’s under huge pressure and isn’t going to solve itself anytime soon.”

The practical offer centres on speed. Members with symptoms or referrals can quickly access diagnostics—MRIs, CAT scans, blood services—and arrange consultations with private consultants. Surgical treatments follow a prescribed list, or patients may return to the NHS if appropriate. “It’s really just about speed of access, speed of diagnosis,” he added – a point that resonates strongly with employers managing long-term sickness absence.

The £15.50 proposition

Affordability is the commercial hook. “We have large national employers and work down to sole traders. Our sweet spot is 100 to 500 employees… £15.50 a month covers the workforce with diagnostic treatments and 24/7 GP access. For individuals it’s £15.85.”

Benenden does not replicate comprehensive private medical insurance. “We don’t cover cardiac, cancer, or brain treatments. We do the tests and diagnoses, but treatment or drugs are generally via the NHS. Early diagnosis is critical, for example with cancer, so patients then enter NHS pathways.”

Benenden supplements care with practical and financial support. But the core design principle remains – limit coverage to keep subscriptions low. Tom said the organisation covers around 10% of NHS procedure codes, representing roughly 85% of procedures by volume. “We try and provide – can’t cover everything. It’s our choice to be affordable and positioned that way.”

With 876,000 members and around £200m in revenue, Benenden is significant. “By lives covered, we’re the largest healthcare provider in the UK.” Its mutual status shapes its financial philosophy: “We do try to make a surplus but limit it to 6%. Anything more, we’re not delivering enough value. Anything less, we can’t invest and sustain the future.”

For a leader from PLC and private equity backgrounds, that requires adjustment. “It’s a reframing: not too much profit, not too little.” Success is now measured differently. “What does a good day feel like? It isn’t about maximising return on equity anymore. It’s about the number of members, sustainability, long-term effective sustainability.”

A politically charged future

Tom does not shy away from the debate around healthcare funding. “There is a two-tier healthcare system today. The NHS is under enormous pressure. It isn’t going to solve itself.”

“If you look at other healthcare systems… the UK cannot afford to continue universal healthcare. Politicians are unlikely to sustain it due to pressures, ageing population, and treatment demands. That’s where the UK needs to go eventually. But politicians aren’t brave enough to say it.”

Benenden’s mutual structure gives it a socially acceptable place in that evolution. “We’re not enriching external shareholders. As a mutual, purpose-led organisation, we feel okay playing a role in that.”

Despite its longevity, Benenden is adopting a challenger mindset. “We’re competing with Bupa, Aviva, AXA, and everyone’s heard of them… even though we’ve been around as long, if not longer.” Marketing spend has risen sharply, with ITV and radio campaigns featuring recognisable faces. Awareness rose from 14% to 22%; consideration doubled.

The economics are tight. “Affordable premiums mean you must sell many subscriptions to recoup marketing investment. But as a mutual, we can take a long-term perspective. It’s as much about retaining members as acquiring them.”

Internally, cultural change is another challenge. “Being a mutual can be safe, slow, maybe conservative. The board hired me to change that culture.” He promotes a “growth mindset”: “We celebrate people who fail. Trying and failing is a way to learn; in the past, failure was avoided.”

The million-member ambition

The strategy crystallises around one headline target. “A million members would be a lovely target. I’m clear not to set a timeline because the board would question it.”

The maths isn’t straightforward. “For every member we acquire, we also lose some. It’s a twin challenge of retention and growth amid huge affordability pressures for households and employers.”

Demographics are also key. “Our average member is well into their 60s. Heritage is the Post Office, civil service; many have aged and benefited over decades. Growth lies with 35–55-year-olds, experiencing NHS access challenges. There’s real opportunity to improve lives, including employees’ lives.”

For Tom, impact drives the business. “There are seven million people are on waiting lists. That seriously impacts quality of life. One of the positive things our team turns up to work for is helping individuals improve their quality of life. That’s rewarding, and we’re proud when we do it.”

If Benenden has been a well-kept secret, Tom’s ambition is clear: make it a mainstream name before pressures on Britain’s healthcare system make this middle-ground model not just attractive, but essential.

A century-old mutual that once flew under the radar is suddenly taking centre stage.
